- New Users
Employees and covered spouses/partners who are new to the Abbott medical plan in 2012 must go to the Registration page to complete the Health Assessment. You will need your Abbott UPI and date of birth. If you are the spouse/partner of an Abbott employee, use their UPI and your own date of birth. - Returning Users
Employees and covered spouses/partners who have previously taken the Health Assessment can login with the username and password created when you previously completed the Health Assessment. Or, follow the prompts on the Health Assessment page to retrieve your username and password. If you need assistance with your username or password, call Healthways at (877) 292-1373 (Monday – Friday, 8:00 a.m. to Midnight ET, and Saturday 8:00 a.m. to 5:00 p.m. ET).
Talk to a Health Coach
After you complete the Health Assessment, you'll receive a personalized report that identifies your health risks and offers tips for improving – so you can be your best at work, at play and with your family. You'll learn about tools and resources available to you and your family. You may also have the opportunity to get free, confidential, one-on-one health coaching from one of our dedicated health coaches who can help you make lifestyle changes to improve your health.
Your privacy is always protected!
All of the information you enter when taking the Health Assessment is completely confidential. Abbott will not have access to your personal health information. The Health Assessment and Health Coaching are administered by Healthways, an independent company, and personal results are never reported to Abbott.
